Abstract:
Sa kaeo Crown Prince hospital are having problems with diabetes being treated a lot
throughout the year. Social Medicine department undertakes a diabetes screening program and
been improving health behavior for people at risk. To reduce the number of diabetic patients.
This study aims to track and evaluate those projects that will be implemented in 2016. Based on
Stake's evaluation model. The sample consisted of 4 staff responsible, 280 people involved and
120 people at risk. Data were collected from official document. Send a questionnaire to staff
responsible, People involved and the people at risk. Between April 2017. And data analysis by
frequency distribution Number and percentage, mean, standard deviation, and t-test statistic.
This follow-up study was found that diabetic screening and improving health behavior
among people at the risk of Social Medicine department. There were 4 staff responsible, 3 related
agencies. The budget was around one hundred fifty thousand baht. There were 5,034 people or
70.6 percent of them who were screening. Of these, 39 were diabetic or 0.7 percent was screened
and 1,126 health behaviors were modified. However, 724 or 64.2 percent of those who had to
undergo improve health behavior. The people at risk asked 120 people. Service access of these
people in overall was 81.9 an average score percent (

diabetes at average score was 78.1 percent and satisfied with the improve health behavior an

percent. And the staff responsible for the operation was satisfied with an average score of 85.8 percent. When comparing the results with the target, it was found that Service access of people
and satisfaction of staff responsible and people involved was passed the criteria 80.0 percent.
